THE ROOM</p><p>In medicine, they call it an M&amp;M Conference - Morbidity and Mortality. It&#8217;s where doctors gather to discuss what went wrong. The unofficial rule is to be &#8220;blameless.&#8221; The actual rule is to blame the system, the protocol, the patient&#8217;s &#8220;non-compliance&#8221; - anything but the surgeon who held the scalpel. I&#8217;ve sat in those rooms. The air is thick with the stench of self-preservation, a noxious perfume worse than any formaldehyde. The goal isn&#8217;t to find the truth. THE RITUAL</p><p>The postmortem is not a diagnostic procedure. It is an institutional exorcism. A ritual designed to conjure a scapegoat and sacrifice it on the altar of The System.</p><p>Psychologically, it provides sanctuary from personal culpability. By blaming the protocol, the software, the &#8220;unforeseeable circumstances,&#8221; the individual actor achieves absolution without penance. It&#8217;s the organizational equivalent of a toddler pointing at the dog and saying, &#8220;He broke it.&#8221; It allows professionals to maintain their self-image as infallible experts while presiding over predictable, repeating failures.</p><p>Institutionally, it&#8217;s a masterpiece of risk management. The System is the perfect defendant. It can&#8217;t be fired. It can&#8217;t be sued. It can&#8217;t feel shame. You can issue a &#8220;corrective action plan&#8221; that nobody will ever read, close the ticket, and declare the problem solved - until the exact same failure happens again next quarter. The ritual doesn&#8217;t fix the bug. It makes the bug a feature of the operating system, renamed &#8220;lessons learned.&#8221;</p><p>III. THE TEMPLATE</p><p>This is not an isolated pathology. The template is the same across sectors.</p><p>Medicine: The M&amp;M Conference. Blame the EMR, the nurse&#8217;s handoff, the sepsis protocol.</p><p>Corporate: The Incident Report. Blame the legacy code, the third-party vendor, the &#8220;aggressive&#8221; deadline.</p><p>Law Enforcement: Internal Affairs. Blame the &#8220;lack of clear policy,&#8221; the suspect&#8217;s &#8220;non-compliance,&#8221; the fog of war.</p><p>Pharma: Adverse Event Reporting. Blame the patient&#8217;s co-morbidities, the &#8220;off-label&#8221; use, the statistically insignificant side effect.</p><p>Different costumes, same performance. The goal is never to identify the single point of failure. The goal is to distribute the failure so thinly across the organism that it becomes undetectable. A systemic cancer the organization refuses to biopsy.</p><p>IV. THE FUNCTION</p><p>Let&#8217;s be precise about the function of the official postmortem. It is not a tool for learning. It is a tool for ending learning.</p><p>The report is the final word. Its existence is proof that the incident has been &#8220;addressed.&#8221; It is a legal and cultural full stop. Its purpose is to close the case, not to open an inquiry. It&#8217;s the institutional equivalent of a witness saying &#8220;I have nothing more to add&#8221; - when the entire testimony has been a carefully rehearsed lie.</p><p>The report is a tombstone, and what they bury inside it is the truth. It creates the permanent, searchable, citable false record that prevents any future real investigation. It is a shield against accountability, constructed entirely from recycled bureaucracy.</p><p>V. THE REAL FINDINGS</p><p>An honest postmortem would require three things institutions are structurally incapable of providing:</p><p>A Perpetrator. Not a &#8220;contributing factor.&#8221; A person with a name who made a decision that was negligent, arrogant, or stupid. A scapegoat is a coward&#8217;s substitute for a culprit.</p><p>A Confession of Incompetence. An admission that the hierarchy, the culture, or the very premise of the organization is flawed. Not a &#8220;process gap&#8221; - a fundamental design flaw. This is a suicide note for the brand, and no executive will ever sign it.</p><p>Restitution Without Audience. A remedy that doesn&#8217;t involve a press release. A quiet, expensive, legally binding act of penance. The kind performed when you&#8217;re actually sorry - not when you&#8217;re sorry you got caught.</p><p>These cannot be produced by design. The institution&#8217;s prime directive is its own survival. An honest postmortem is an autoimmune disease - the body attacking the only thing that could save it.</p><p>THE CASE STAYS OPEN.</p><p>The Postmortem Report files the report the institution never will. Every incident. Every pattern.
